2. Complete the application form.
3. Be sure to provide signatures as required throughout the form.
4. Retain a copy of the completed form for your records.
5. Mail or fax the completed insurance form to Cruise & Tour with you travel reservation.

SPECIAL NOTE: A COPY OF A VALID US PASSPORT WITH AN EXPIRATION DATE OF SIX MONTHS OR MORE BEYOND THE RETURN DATE OF THIS TOUR MUST BE INCLUDED WITH YOUR RESERVATION FORM. THE NAME ON YOUR PASSPORT MUST MATCH THE FIRST AND LAST NAME ON YOUR RESERVATION. NON-U.S. CITIZENS MUST CONSULT THE APPROPRIATE CONSULATES TO DETERMINE IF ANY VISAS ARE NEEDED AND ARE RESPONSIBLE FOR OBTAINING ALL VISAS AND ENTRY DOCUMENTS.
